Passport Day In Dallas 2016

The Dallas Passport Agency will open its doors on Saturday, September 17th, 2016 from 9:00am until 3:00pm for a special Passport Day.  On Passport Day, no appointments are needed to apply for a passport, and customers can request routine or expedited service.  Customers are encouraged to complete applications online and bring unsigned to the agency along with the following items:
​Adult (16 years of age or older) First Time Instructions

• Fil out, in black ink (do not sign), Form DS-11: Application for a U.S. Passport
• One color Passport Photograph
• Evidence of U.S. Citizenship
• A Photocopy of your Photo Identification (front and back)
• $135 fee (preferred method of payment is credit car, money order or exact cash)

Minor (15 years of age and under) First Time Instructions:

• Fill out in black ink (do not sign), Form DS-11:  application for a US Passport
• One Color Passport Photograph
• Evidence of citizenship (when parents/guardians submit a child's previous passport or naturalization certificate, they must also submit a photocopy of a birth certificate or foreign birth certificate with an official translation in English).
• Parents or guardians Photo Identification and photocopy (front and back)
• $105 fee (preferred method of payment is credit card, money order or exact cash)
• If the minor has two parents/guardians, but one is absent, the applying parent must submit a notarized DS-3053 from the non-appearing parent along with a photocopy (front and back) of his/her identification.
